{"id":3937,"date":"2020-08-13T07:35:00","date_gmt":"2020-08-13T07:35:00","guid":{"rendered":"https:\/\/aspree.org\/aus\/?p=3937"},"modified":"2021-08-02T07:04:55","modified_gmt":"2021-08-02T07:04:55","slug":"aspirin-and-major-gastrointestinal-bleeding-risk-in-healthy-older-adults","status":"publish","type":"post","link":"https:\/\/aspree.org\/aus\/aspirin-and-major-gastrointestinal-bleeding-risk-in-healthy-older-adults\/","title":{"rendered":"Aspirin and major gastrointestinal bleeding risk"},"content":{"rendered":"\r\n<h2 class=\"wp-block-heading\">New analysis identifies potential risk of major GI bleeding in older adults<\/h2>\r\n\r\n\r\n\r\n<p>&nbsp;<\/p>\r\n\r\n\r\n\r\n<p>A closer analysis of data from the ASPREE trial provides doctors, for the first time, with information about how much aspirin may increase the risk of gastrointestinal (GI) bleeding in healthy adults aged 70 and over.<\/p>\r\n\r\n\r\n\r\n<p>The Monash University-led analysis, published in top gastroenterology journal\u00a0<a href=\"https:\/\/gut.bmj.com\/wp-content\/early\/2020\/08\/02\/gutjnl-2020-321585\" target=\"_blank\" rel=\"noreferrer noopener\">Gut,<\/a> followed 19,114 ASPREE participants over almost five-years and recorded 264 incidents of major GI bleeding. Bleeding is a well-known side-effect of aspirin.<\/p>\r\n\r\n\r\n\r\n<p>Of the 264 incidents of both upper and lower GI bleeding, 162 occurred in those taking aspirin daily and 102 in the placebo group (no aspirin). No deaths were recorded because of a GI bleed in the aspirin group.<\/p>\r\n\r\n\r\n\r\n<p>This is the largest analysis of GI bleeding in older adults as part of a randomised controlled trial \u2013 with half the participants taking a placebo and the other half a daily low-dose (100mg) of aspirin.<\/p>\r\n\r\n\r\n\r\n<p>The international team of researchers, led by Australian gastroenterologist and clinical epidemiologist\u00a0<a href=\"https:\/\/www.monash.edu\/medicine\/sphpm\/about\/staff\/academic\/mahady\" target=\"_blank\" rel=\"noreferrer noopener\">Dr Suzanne Mahady<\/a>\u00a0from the School of Public Health and Preventive Medicine, found the risk of major gastrointestinal bleeding \u2013 requiring hospitalisation or blood transfusions or surgery \u2013\u00a0 increased by around 60% with daily aspirin use. This risk continued to increase significantly with age.<\/p>\r\n\r\n\r\n\r\n<p>The analysis also found that smoking, hypertension, obesity, chronic kidney disease and the use of anti-inflammatory drugs also increased bleeding risk, and the combination of these risk factors in association with aspirin put people at high risk of complications.<\/p>\r\n\r\n\r\n\r\n<p>Many people around the world take aspirin as a preventive medication to keep them healthier as they age. The ASPREE trial in Australia and the US was the first trial to examine the benefits and risks of low-dose aspirin in otherwise healthy older individuals.<\/p>\r\n\r\n\r\n\r\n<p>This analysis adds to growing evidence surrounding the caution of using daily low-dose aspirin without a medical need to do so.<\/p>\r\n\r\n\r\n\r\n<p>Dr Mahady said these new data might assist patients and their clinicians to make informed decisions about the use of aspirin.<\/p>\r\n\r\n\r\n\r\n<p>\u201cWe know that the most common side-effect of aspirin is bleeding, and that older people are at higher risk, but until now we haven\u2019t been able to accurately quantify that risk,\u201d Dr Mahady said.<\/p>\r\n\r\n\r\n\r\n<p>\u201cNow we have a much clearer picture of the additional risk of bleeding with aspirin and that taking aspirin when you have additional risk factors including hypertension, a history of smoking, kidney disease or with anti-inflammatory medication could be quite dangerous.<\/p>\r\n\r\n\r\n\r\n<p>\u201cPrescribing doctors may now use these data to assess the bleeding risk of aspirin use among their older patients, and may alternatively target the modifiable risk factors to reduce harm.\u201d<\/p>\r\n\r\n\r\n\r\n<p>Dr Mahady said all patients should follow the advice of their doctor about their daily use of aspirin.
